The following pattern is my original design.

MATERIALS:
Hook: Size G
Yarn: Simply Soft Yarn (4 ply worsted weight)
Yarn Needle
Stitches used: ch, sl st, sc, dc.
Difficulty: EASY
ROW 1: Ch 20, sc in 2nd ch from hk and in each rem ch.
ROW 2: * Sc in next st, dc in next st. Rep from * around. Ch 1, turn.
ROW 3: Sc in next st, * sc in next dc, dc in next sc. Rep from * across to last 2 sts. Sc in each of last 2 sts.
Rep Rows 2&3 until you reach the head circumference. In other words... keep alternating Rows 2 and 3 until the piece measures 6-1/2" to 7".
FASTEN OFF, leaving a 12-14" yarn tail.
Thread the yarn needle with the tail left after you fastened off. Sew Row 1 to the last row you worked. Now weave the yarn in and out the ends of the rows on that end and draw the yarn tight to close the end.
FASTEN OFF.
FINISHING: Add a pom pom on the closed end and fold the open end to form a band.

HOW TO MAKE A POM POM:
Wind yarn around a piece of 3-4 inch wide cardboard (width depends on how big you want the pom pom) about 40-50 times (how many times you wrap it depends on how full you want the pom pom to be). Slide the wrapped yarn off of the cardboard and tie a long piece of yarn around the center of the bunch. Cut both ends of the folded yarn so that you have a bunch of individual pieces. This should begin to look like a pom pom. Now hold the long yarn strands and shake it over a trash can then use scissors to trim it till it is round. Now use the long strands to tie it to your project (i.e. hat).
You may need to experiment with the size (how big for the cardboard and how many times you wrap) to get the size you want but this is an easy method to use.
